While DOOM has always been more about fast-paced demon slaying than deep lore, DOOM: The Dark Ages proves that the franchise can do both. This time around, the story, worldbuilding, and detailed codex entries add a surprising amount of depth. If you’re hunting down every collectible, the 26 codex entries scattered across 22 chapters are a must—especially if you’re aiming for that Lore Nerd Trophy / Achievement.
Here’s a full breakdown of where to find each one:
All Codex Entry Locations in DOOM: The Dark Ages
Chapter 1
- Doom Slayer: Behind one of the four Demonic Portals, look for a climbable wall that leads to this entry.
- Village of Khalim: After dropping the red forcefield near the chapter’s end, find a climbable wall on your right. Codex is near some Sentinel soldiers.


Chapter 2

- Hebeth: After pulling a lever and entering a green-lit doorway, hop over crates to your left to reach a hidden area with the codex.
Chapter 4

- Sentinel Barracks: Just before the Dragon’s Den area, take a left path before the Sentinel Shrine to find this entry and some gold.
Chapter 5
- Deag Loric: After finding the Cycler, double back under the metal railing you dropped from to find the codex.
- The Holy City of Aratum: After grabbing the Super Shotgun, break a nearby metal cross, move a block, and circle back through a broken window to reach the codex down a hallway.


Chapter 6
- Siege – Part 1: Near the Atlan-Titan fight, jump on debris piles to grab this codex.
- Kreed Maykr: Near a Sentinel Shrine and shallow water, find a climbable wall with this codex (close to the Mancubus toy).


Chapter 7

- Siege – Part 2: Use the secret key to open a gate, defeat two shield enemies, and grab the codex.
Chapter 8

- Abyssal Forest: During the barracks section, find breakable vines on the right near a row of shield enemies. Codex is behind them.
Chapter 9

- Ancestral Forge: Near the spectral wolf statues, drop to a wooden platform to find it.
- King Novik: In the moving platforms section, spot a red cross above. Break it with your shield, then raise a platform and backtrack to reach the codex using jump boosts.

Chapter 10

- The Forsaken Plains: Near a Sentinel Shrine and a spiked falling plate, follow the gold trail to this codex.
- Prince Ahzrak: Use the secret key to access a gated area near the fallen Atlan. Codex is inside.

Chapter 12

- Sentinel Command Station: After the vent section, hug the right wall to find gold and this entry.
Chapter 14

- Spire of Nerathul: Found on a ledge near the drained water used to reach a Stasis Emitter.
- Serrat: Before activating the elevator with the green platform (point of no return), check the right side for a small path and the codex.

Chapter 15

- City of Ry’uul: During the portal activation sequence, find a ledge on one side of the map. Climb the wall above it to reach the codex.
Chapter 16

- The Witch: Look for a blue gear visible through a hole near the center of the map. Hit it to lift a grate and access the codex.
Chapter 17

- Temple of Lomarith: In the underwater section, swim into a nearby upturned shipwreck. The codex is inside.
Chapter 18

- Commander Thira: Found in a wall gap in the beast’s belly during the “Destroy the Organ” section. Take the upper route and push through the stomach acid to reach it.
Chapter 19

- Harbor of Souls: Out in the open to the left of the main path. Super easy to spot.
- Hell Priests: After the moving platforms and flame traps, turn around at the point of no return and follow the left path.

Chapter 20

- Resurrection: Behind the key door near the yellow key ledge. Also where the Cosmic Baron toy is hiding.
- Commander Valen: After mounting the dragon and landing on the Kreed ship, turn around and check the edge of the landing pad.

Chapter 22

- Marok: After charging through the green gate, don’t jump into the battlefield. Instead, follow the right-hand path to find this final codex. Collecting it (along with the others) unlocks the Lore Nerd Trophy / Achievement.
If you’re all about lore, secrets, and 100% completion, these codex entries are the way to go. Plus, some of the backstory and world details they unlock are actually pretty awesome.
