The catch is that you will find the safe long before the game gives you its combination. The code file is locked behind a story progression gate on the 2nd floor of the East Wing, in a room tied to the Chunk boss encounter. But you do not need the file to open the safe — if you already know the code, you can crack it open as soon as you walk into the Examination Room.

Examination Room Safe Code in Resident Evil Requiem
Here is the Examination Room safe combination for every difficulty in RE9:
Casual and Standard Difficulty
- Right 30 — Turn the dial to the right until it reads 30
- Left 10 — Without pressing confirm, turn the dial to the left until it reads 10
- Right 50 — Turn the dial back to the right until it reads 50
- Press X (PlayStation) / A (Xbox) / Confirm to open the safe
Insanity Difficulty
On Insanity mode, all safe codes in Resident Evil Requiem change. The Examination Room safe code on Insanity is:
- Left 50 — Turn the dial to the left until it reads 50
- Right 30 — Turn the dial to the right until it reads 30

Once inside the East Wing, you will reach the Examination Room as part of the story. The safe itself is tucked inside a cabinet on the shelves, to the right of the examination table. The room is relatively safe compared to other safe locations in the Care Center — there are no enemies guarding it directly, so you can take your time entering the combination.
What Is Inside the Examination Room Safe
Opening the Examination Room safe gives you 3 Antique Coins. Combined with the 3 coins from the Bar & Lounge safe, these 6 coins are critical for purchasing permanent upgrades at the Parlor in the West Wing.
At the Parlor, Antique Coins can be exchanged for:
- Hip Pouch — Expands Grace’s inventory capacity
- Stabilizer — Permanently increases Grace’s weapon damage
- Health upgrades — Boosts maximum health
Since the Bar & Lounge and Examination Room safes are the only two that contain Antique Coins in the Care Center, opening both is essential if you want to buy all available upgrades before leaving Rhodes Hill. - Enter the blood-stained locker room — The room just beyond the Lead Researcher’s Office contains the code file. This is the same area where the Chunk boss first appears and where you pick up the Level 1 ID Wristband.
- Check the locker — The Examination Room Safe Code file is sitting on top of one of the lockers in this room.
The file confirms the combination: Right 30, Left 10, Right 50.
Note: You will likely encounter the Chunk boss here before you can safely grab the file. After the Chunk retreats, backtrack to the locker room to collect the code along with the ID Wristband.
Tips for the Examination Room Area
- Open the safe early. Since there are no enemies guarding the Examination Room safe directly, it is one of the safest safes to crack in the entire game. Do it as soon as you enter the East Wing.
