COD Modern Warfare 4 Campaign Trophy & Achievement List – All 18 Trophies Explained

COD Modern Warfare 4 Campaign Trophy Achievement List

    TL;DR

    • MW4 has 18 total trophies and achievements — all in the campaign, none in multiplayer or DMZ
    • 8 are Gold, 10 are Silver — no Platinum on PS5, no Bronze anywhere
    • Total Gamerscore on Xbox: 1,000
    • You need a minimum of 2 playthroughs — Realism difficulty is locked until you finish the story once
    • Nothing is permanently missable — mission select lets you replay any mission
    • Hardest trophy: Was It Worth It? (complete the campaign on Realism)
    • Revenge and Pacifist cannot be done in the same run of Assassins
    • Estimated time to 100%: 15+ hours

COD Modern Warfare 4 keeps its trophy and achievement list tight. Every single one of the 18 trophies is tied to the campaign. No multiplayer grind. No DMZ requirements. Just the story mode, done well.

Some are simple — finish the campaign, stay on a train, shoot some parachuters. Others take real planning, like completing the game on Realism, or finishing the Assassins mission with only one kill. This guide covers all 18 in detail: what they are, which mission they are in, difficulty requirements, and how to unlock each one without wasting a playthrough.

Want a deeper look at two of the trickiest trophies in Mission 3? See our No Stone Unturned achievement guide and No Harm Done achievement guide for full step-by-step breakdowns.

All 18 COD Modern Warfare 4 Trophies and Achievements

The list below covers every trophy in the game, sorted by the order you encounter them in the campaign. Each entry includes the requirement, the tier, and practical notes on how to unlock it.

    Hold the Line — Gold

Requirement: Complete the game on any difficulty.

Finish all 16 campaign missions. You can play on Recruit — the lowest difficulty — and this trophy still unlocks. Completing the story on any difficulty also unlocks Realism mode for your second playthrough. There is nothing tricky here. Just reach the credits.

Was It Worth It? — Gold

Requirement: Complete the game on Realism difficulty.

This is the hardest trophy in MW4. Realism removes the HUD entirely — no minimap, no ammo counter, no health indicator — and turns on friendly fire. Enemy accuracy is higher, they throw more grenades, and you go down fast. You must finish the story once first to unlock this mode. When you tackle Realism, stay behind cover, move carefully, and never rush open areas. If you have completed Veteran in previous COD games, this is manageable. It is a test of patience more than pure skill.

    Skeet Shooting — Silver

    Mission: Mayhem (Mission 1)

Requirement: Kill 10 paratroopers before they land.

When you reach the stadium field in Mayhem, enemies start dropping in by parachute. You need to shoot 10 of them before they hit the ground. All 10 kills need to happen in a single attempt — if too many enemies land, reload the checkpoint and try again. Lowering your FOV in settings gives a bit of extra zoom and helps with tracking the targets as they fall.

    Safety’s On — Gold

Mission: Out in the Cold (Mission 2)

Requirement: Defeat all enemies in the kitchen without firing your gun on Hardened difficulty or higher.

After passing through the green-forest room in Mission 2, you enter a corridor that leads to a kitchen with five enemies. You can play the mission on Recruit up until this point. Just before entering the corridor, switch to Hardened or higher. Make sure you have picked up bottles on your way through — you will need at least four. Inside the kitchen, throw bottles to stagger enemies and finish them with melee takedowns. Work from right to left. Do not fire a single shot or the trophy fails.

    Can’t Touch This — Gold

Mission: Out in the Cold (Mission 2)

Requirement: Defeat Makarov in the alley fistfight on Veteran difficulty or higher without getting hit.

At the end of Mission 2, you chase Makarov through the city and end up in an alley for a hand-to-hand fight. You can play everything leading up to the alley on Recruit. Change difficulty to Veteran or higher just before entering the alley. If your screen flashes red at any point during the fight, reload the checkpoint. There is a checkpoint mid-fight, so a hit after that checkpoint still allows the trophy to unlock. Block his attacks using the left trigger and counter when you see an opening.

    No Stone Unturned — Silver

    Mission: Following Orders (Mission 3)

Requirement: Confiscate all contraband items and read all notes.

There are 3 contraband items and 4 notes spread across three houses in this mission. The achievement pops the moment you read the final note — Note From Mom — inside the cellar of the third house. Search every cabinet in the first house before speaking to anyone. The red lockbox in the first room uses the code 214. Do not rush past any rooms. For the complete location of every item, see our No Stone Unturned guide.

    No Harm Done — Silver

    Mission: Following Orders (Mission 3)

Requirement: Get the defector’s location without hurting any civilians.

This trophy is earned entirely during the interrogation in the first house. You need to question the couple using peaceful dialogue options — do not shoot them, and do not pick the option that causes the wife to flee. The trophy unlocks the moment the husband tells you where the defector is hiding. It can be done in the same run as No Stone Unturned. See our No Harm Done guide for the exact dialogue choices.

    Bullet Train — Silver

    Mission: Hired Gun (Mission 4)

Requirement: Kill 50 enemies while standing on train cars.

The entire Hired Gun mission takes place in and around a train yard. Stay on top of train cars — moving or stationary — while getting kills. The count only tracks kills made while you are physically standing on a train car, not on the ground nearby. Move forward slowly and only advance when no enemies are visible so you do not miss any kills. You should hit 50 before the mission ends if you stay on top of the cars throughout.

    Raining Thunder — Silver

    Mission: Losing Ground (Mission 5)

Requirement: Kill 15 enemies using 3 consecutive mortar rounds in a row.

When the mortar objective comes up in Losing Ground, you need 5 kills per shot across 3 shots in a row. The mission gives you four opportunities to fire at enemy groups. Target the most clustered groups first. If you miss the total after three shots, reload the checkpoint rather than continuing — it is faster than replaying the full mission to reach the mortar again.

    Revenge — Silver

    Mission: Assassins (Mission 8)

Requirement: Kill every enemy after Shin dies on Hardened difficulty or higher.

Shin is your bodyguard and dies early in the mission — you cannot prevent this. After he dies, you need to eliminate every remaining enemy completely before moving to the next area. Do not rush ahead. Near the end, inside the bunker area with door code 9103, do not climb the ladder yet. Go prone and wait for a final wave of four enemies. Kill them all and the trophy unlocks. Note: Revenge and Pacifist cannot be done in the same run. You will need to replay Assassins for both.

    Pacifist — Gold

    Mission: Assassins (Mission 8)

Requirement: Complete Assassins by killing only one enemy.

The one allowed kill happens automatically as a scripted story event — it leads to Shin’s death. After that point, you need to avoid every remaining enemy through stealth. Stick with your other bodyguard Ko through the first half. In the library section, take the door using code 5523. Cut through the kitchen (door code 7632). In the garden, swim under the gazebo to avoid the sniper inside. Reach the backside of the large statue and enter the bunker (code 9103). The trophy unlocks after you go down the ladder and through the door inside.

    Time to Crate — Gold

    Mission: Stranded (Mission 10)

Requirement: Open all 26 Yellow Supply Crates.

Stranded is an open-world mission with no time limit, so you have all the time you need. Yellow Supply Crates are scattered across the map in buildings, bunkers, and locked containers. Two crates require extra attention — one is carried on a moving train and another on a patrol truck. You will need to intercept or stop them. This trophy pairs naturally with Scavenger and Noteworthy since you are exploring the same areas.

    Scavenger — Gold

    Mission: Stranded (Mission 10)

Requirement: Repair everything in Base Camp.

Base Camp has 12 items to repair or craft — four weapons and eight attachments or pieces of equipment. The total cost is 195 Supplies. You collect Supplies by opening Yellow Supply Crates, looting backpacks on the ground, and grabbing supply drops that fall periodically. If you are already doing Time to Crate and Noteworthy at the same time, you will likely gather enough Supplies naturally without needing to hunt specifically for them.

    Incoming! — Silver

    Mission: Stranded (Mission 10)

Requirement: Destroy every reinforcement vehicle at the SAM Site before passengers unload.

During the SAM Site objective in Stranded, enemy reinforcements arrive in waves — 8 trucks and 1 tank. You need to destroy all of them before any enemy gets out. Stock up on bomb drones from green crates in the area before the assault begins. Place proximity mines on or near the bridge where vehicles come from. The tank needs two drone hits. Everything else goes down with one. Keep watch on your supply of drones throughout the fight.

    Long Shot — Silver

    Mission: Counter Attack (Mission 12)

Requirement: Snipe all 4 enemies firing the mortars.

In Counter Attack, you defend from a rooftop. Two mortar teams of two soldiers each appear during the mission. They arrive one team at a time. After you destroy a tank below and to your left, check the roof above it for the first team. You need both soldiers down quickly — if one escapes, reload the checkpoint. After neutralising the first team and killing a sniper, watch the roof to the left for the second team. Both teams eliminated means the trophy unlocks.

    Never Forgotten — Silver

    Mission: Escape (Mission 13)

    Requirement: Recover the Dog Tags.

Early in Mission 13, you pass through a room guarded by a soldier. Wait for the soldier to leave, then go inside and pick up a note from the room. Later in the mission, after leaving the barracks room, immediately enter the room to your right. Open the safe using code 8410. The Dog Tags are inside. The trophy unlocks as soon as you take them.

    Trench Broom — Silver

    Mission: Entrenched (Mission 14)

Requirement: Kill 15 enemies with a shotgun before securing the Target Building.

Equip the secondary shotgun at the very start of Entrenched. Use it throughout the trench sections at the beginning of the mission. The trench encounters give you plenty of close-range enemies to work with. Make sure you have at least 10 kills before an ally saves you from an ambush near the end of the trenches — only a handful of enemies appear after that before you reach the target building. Hit 15 before entering and the trophy is yours.

    Noteworthy — Gold

Requirement: Read 50 different notes across the campaign.

There are 64 notes in the full campaign. You only need 50. The vast majority — 58 of the 64 — are found in Mission 10: Stranded, which is an open-world mission with no time pressure. If you explore Stranded thoroughly while going for Time to Crate and Scavenger, you will almost certainly hit 50 without needing to backtrack through earlier missions. The 4 notes in Following Orders do not count toward this total — they are tracked separately for No Stone Unturned.

    Which Trophies Have Difficulty Requirements?

Most MW4 trophies work on any difficulty. But four of them need a specific setting:

    • Safety’s On — Hardened or higher (Mission 2)
    • Can’t Touch This — Veteran or higher (Mission 2)
    • Revenge — Hardened or higher (Mission 8)
    • Was It Worth It? — Realism (full campaign)

The good news is that for Safety’s On, Can’t Touch This, and Revenge, you can play the rest of the mission on a lower difficulty and only switch when you reach the specific moment the trophy requires. You do not need to run the whole mission on Hardened or Veteran just for these.

COD Modern Warfare 4 No Harm Done Achievement
COD Modern Warfare 4 No Harm Done Achievement

Which Trophies Cannot Be Done Together?

Only one pair of trophies directly conflicts:

    • Revenge requires killing every enemy after Shin dies
    • Pacifist requires finishing Assassins with only one kill total

These two are mutually exclusive in the same run. You need to replay Mission 8: Assassins twice — once for each. Use mission select after finishing the story. Every other trophy in the list can be combined however you like.

    Recommended Trophy Completion Order

The cleanest way to get all 18 trophies without wasted runs:

    • First playthrough (any difficulty): Focus on Skeet Shooting, No Stone Unturned, No Harm Done, Bullet Train, Raining Thunder, Pacifist, Time to Crate, Scavenger, Incoming!, Long Shot, Never Forgotten, and Trench Broom. Grab Noteworthy during Stranded. This gets you the story completion trophy and unlocks Realism.
    • Replay Assassins once more: Do Revenge on Hardened. This is the one trophy that conflicts with Pacifist, which you should have already done in your first run.
    • Mission select for Out in the Cold: Do Safety’s On on Hardened and Can’t Touch This on Veteran if you did not grab them during your first run.
    • Second full playthrough on Realism: Complete the whole campaign again for Was It Worth It?

Is There a Platinum Trophy in MW4?

No. There is no Platinum trophy for Modern Warfare 4 on PS5. The MW4 achievements are registered as an additional trophy set under Call of Duty: Modern Warfare II rather than as a standalone game on your profile. This means the maximum you can earn is 100% completion of the MW4 trophy set — 18 trophies, no Platinum. On Xbox, the maximum is 1,000 Gamerscore.

    Frequently Asked Questions

How many trophies does COD Modern Warfare 4 have?

Modern Warfare 4 has 18 trophies and achievements in total — 8 Gold and 10 Silver. There are no Bronze trophies and no Platinum on PS5. On Xbox, completing all 18 earns you 1,000 Gamerscore.

Are there any multiplayer or DMZ trophies in MW4?

No. Every single trophy and achievement in Modern Warfare 4 is tied to the campaign. You do not need to play multiplayer or DMZ at all to reach 100% trophy completion.

Can you miss any trophies in MW4?

No. Nothing is permanently missable. Once you complete the campaign, mission select becomes available and lets you replay any mission freely. If you miss a trophy on your first run, just go back to that mission and try again.

How many playthroughs do you need for 100% in MW4?

A minimum of two. Your first run can be on any difficulty and should cover most combat and collectible trophies. Your second run needs to be on Realism for the Was It Worth It? trophy. You will also need to replay Assassins twice since Revenge and Pacifist cannot both be earned in the same run of that mission.

What is the hardest trophy in Modern Warfare 4?

Was It Worth It? is the hardest — it requires completing the entire campaign on Realism, which plays like Veteran but without any HUD and with friendly fire enabled. Can’t Touch This (beat Makarov without getting hit on Veteran) is the second most demanding, though it only applies to a short fistfight rather than a full mission.

Do the notes in Following Orders count toward Noteworthy?

No. The 4 notes collected in Following Orders for the No Stone Unturned achievement are tracked separately and do not count toward Noteworthy, which requires reading 50 of the campaign’s 64 notes. The bulk of those 64 notes are found in Stranded (Mission 10).

Can Revenge and Pacifist be done in the same run of Assassins?

No. Revenge requires killing every enemy in the mission after Shin dies, while Pacifist requires finishing with only one kill total. They are completely incompatible. You must replay Assassins through mission select to earn both.

What is the safe code in Escape for Never Forgotten?

The safe code is 8410. After leaving the barracks room in Mission 13, turn right and enter the room immediately on your right. Open the safe with that code to recover the Dog Tags and unlock the trophy.
