TL;DR
- The Ordell Transfusion Door comes right after you open the blood bag wall door in Medical Center — Different.
- The door has a large circular dial with patient initials around the outside rings and ORDELL in the center.
- Check Simon’s Medical Wristband in your inventory — his blood type is A+.
- Blood types compatible with A+ are: O-, O+, A-, and A+.
- Three patients on the dial have compatible blood types: H.M. (O-), T.F. (O+), and S.W. (A+).
- Rotate the outer rings so that only H.M., T.F., and S.W. connect their pipes to the Ordell center.
- Highlight the ORDELL panel and interact with it. The blood flows in and the door opens.
- If the blood fills then drains back out, one ring is slightly off. Adjust them one at a time — do not reset from scratch.
What Is the Ordell Transfusion Door Puzzle?
The Ordell Transfusion Door is the final puzzle inside Medical Center — Different, the Otherworld version of St. Amelia Medical Center in Chapter 8. You reach it immediately after placing the three blood bags on the wall and hitting the switch to open the reinforced double doors.
What waits on the other side is a large steel door with a circular wall-mounted dial. It has patient initials arranged around the outside of several rotating rings, with the word ORDELL in the center panel. There are also blood type compatibility plaques on the walls of the room.
At first glance it looks overwhelming. It is not. Once you understand the logic, it takes less than a minute to solve.
For the full walkthrough of everything before this point — finding all five blood bags, opening the S.W. and D.P. doors, and placing the bags on the wall — see our Silent Hill Townfall Blood Transfusion puzzle guide.
How the Ordell Transfusion Door Works
The dial is built around one simple idea: Simon Ordell needs a blood transfusion. The door will only open when blood types compatible with Simon’s are routed into the center Ordell container.
Every patient initial on the dial represents a real blood bag you collected during this section. Each one has a different blood type. Your job is to figure out which three patients have blood types that are safe to give to Simon — and then connect those three to the center.
The other initials on the dial are there to mislead you. Do not connect them. They are incompatible with Simon’s blood and will cause the puzzle to fail.
Step 1 — Find Simon’s Blood Type
Open your inventory and look at the Medical Wristband item. Simon has been wearing it the entire game. It shows his blood type as A+.
This is the key to the whole puzzle. Everything else follows from it.
Step 2 — Read the Blood Type Plaques on the Wall
In the room with the Transfusion Door, look at the plaques mounted on the walls. They show a blood type compatibility chart — which blood types can safely donate to which recipients.
Find the A+ entry. It shows that a person with A+ blood can safely receive blood from these four types:
- O-
- O+
- A-
- A+
These are the only blood types that will not harm Simon. Any other type on the dial is incompatible and must be left disconnected.
Step 3 — Match the Patients to Their Blood Types
Now look at the patient initials on the rotating dial and cross-reference their blood types. You can check the blood bag labels on the wall behind you from the previous puzzle for each patient’s blood type.
Going through all the patients on the dial, only three have blood types that fall within the A+ compatible list:
- H.M. — Hannah McGregor — blood type O- — compatible
- T.F. — Thomas Fergusson — blood type O+ — compatible
- S.W. — Sarah Watson — blood type A+ — compatible
The remaining patients on the dial have blood types that are not compatible with A+. Leave them disconnected. Routing any of them to the center will cause the puzzle to fail.
Step 4 — Rotate the Rings to Connect H.M., T.F., and S.W.
The dial has several concentric rotating rings. Each ring has blood pathway lines running through it. When a ring is in the right position, the pathway from a patient initial will connect through to the center Ordell panel.
Rotate the outer rings one by one until the pipe lines from H.M., T.F., and S.W. all form a continuous path leading into the Ordell center. The other initials on the dial should have broken or disconnected lines — no continuous path reaching the center.
You do not need to connect every initial. You only need these three. Leave everything else unconnected.
Step 5 — Interact With the ORDELL Panel
Once all three lines — H.M., T.F., and S.W. — connect cleanly to the center, highlight the ORDELL panel in the middle of the dial and press interact (X on PlayStation, A on Xbox).
The blood flows into the central chamber. The door opens. The Ordell Transfusion Door puzzle is done.
What to Do If the Blood Drains Back Out
Some players hit interact and see the blood start filling — then drain back out. This means one of the rings is a single notch off. The connection looks right but is not quite aligned.
Do not reset everything from scratch. Instead:
- Adjust the rings one at a time. Move each ring by one position and try the ORDELL button again.
- Check that all three lines — H.M., T.F., S.W. — run from the outside of the dial all the way into the center without any gap or break.
- Make sure no other initials have accidentally connected. Even a partial connection from an incompatible patient can block the puzzle.
A single small adjustment is usually all it takes. Patience here is faster than starting over.
Why Is It Called ORDELL?
Ordell is Simon’s surname. The character you play throughout Silent Hill Townfall is Simon Ordell. His name has been on the Medical Wristband in your inventory since the start of the game.
The puzzle is designed around the idea that Simon himself needs the blood transfusion. The door opens because the compatible blood flows into the container marked with his name. It is one of the more clever puzzles in the game once you understand the connection.
The Full Blood Type Logic Explained
If you want to understand why these three specific patients are the answer, here is the full logic:
- O- is the universal donor. Hannah McGregor’s O- blood is safe for everyone — including A+ recipients like Simon. H.M. connects.
- O+ can donate to A+. Thomas Fergusson’s O+ blood is compatible with Simon’s A+ type. T.F. connects.
- A+ can donate to A+. Sarah Watson’s blood type matches Simon exactly. S.W. connects.
- Every other patient on the dial has a blood type — such as B+, B-, or AB — that cannot safely be given to an A+ recipient. Those lines must stay broken.
You do not need to memorise blood type science to solve this. The compatibility chart on the wall in the room tells you exactly what A+ can receive. Cross-reference with the patient initials and the answer becomes clear.
What Comes Before This Puzzle
To reach the Ordell Transfusion Door you must first complete the blood bag wall puzzle in the same chapter. That involves:
- Placing Sarah Watson in the first left wall slot
- Placing Matthew Clark in the second left wall slot
- Placing George McEwan in the right wall slot
- Hitting the switch beside the double doors to open the reinforced door
The Transfusion Door is what you find on the other side of those double doors. For the full walkthrough of the blood bag stage and all five blood bag locations, see our Blood Transfusion puzzle guide.

Frequently Asked Questions
What is the Ordell Transfusion Door solution in Silent Hill Townfall?
Rotate the rings on the circular dial so that the pipe lines from H.M. (Hannah McGregor, O-), T.F. (Thomas Fergusson, O+), and S.W. (Sarah Watson, A+) all connect to the Ordell center. Leave all other initials disconnected. Then interact with the ORDELL panel in the middle to open the door.
Why does the puzzle use the name ORDELL?
Ordell is Simon’s surname — the main character you play in Silent Hill Townfall. His full name, Simon Ordell, is on the Medical Wristband in your inventory. The puzzle represents a blood transfusion being routed into Simon himself. Only compatible blood types can flow into his container.
What is Simon’s blood type in Silent Hill Townfall?
Simon Ordell’s blood type is A+. You can find this by checking the Medical Wristband item in your inventory. This tells you which patients’ blood types are compatible and which pipes need to connect to the Ordell center on the dial.
Which patients connect to ORDELL on the Transfusion Door?
The three patients with blood types compatible with Simon’s A+ are H.M. (Hannah McGregor, O-), T.F. (Thomas Fergusson, O+), and S.W. (Sarah Watson, A+). These are the only three pipe lines that should connect to the center. All others must stay disconnected.
What blood types are compatible with A+ in Silent Hill Townfall?
The blood type compatibility plaques in the Transfusion Door room show that A+ recipients can safely receive blood from O-, O+, A-, and A+ donors. Of all the patients on the dial, only H.M. (O-), T.F. (O+), and S.W. (A+) fall into this list.
Why does the blood fill and then drain back out?
If the blood starts flowing and then reverses, one of the rings is a single position off. The line looks connected but has a small gap. Adjust each ring one notch at a time and retry the ORDELL button after each adjustment. Do not reset the whole puzzle — one small tweak is almost always all it needs.
Where is the Ordell Transfusion Door in Silent Hill Townfall?
It is inside Medical Center — Different, the Otherworld version of St. Amelia Medical Center in Chapter 8. You reach it by going through the reinforced double doors that open after you correctly place Sarah Watson, Matthew Clark, and George McEwan’s blood bags on the wall and hit the switch.
What happens after I solve the Ordell Transfusion Door?
After interacting with the ORDELL panel and the blood flows through, the final door opens and you exit the Medical Center — Different section. The Chapter 8 puzzle sequence ends here and the story continues with Simon outside the altered hospital.
Do I need to connect all the initials on the dial?
No. You only connect three — H.M., T.F., and S.W. The other initials on the dial represent patients with incompatible blood types. They are there to make the puzzle look more complex than it is. Leave them disconnected or the puzzle will not complete correctly.
