The Witcher 3 Remastered: How to Heal — Every Method Explained

    By Krushna Vasudeva · September 29, 2026 · Updated October 16, 2026 · 19 min read
The Witcher 3 Remastered How to Heal
    • TL;DR
    • There are six main ways to heal in The Witcher 3 Remastered: food and drink, Swallow potion, meditation, Quen sign, decoctions, and alchemy skills.
    • Food and drink are your primary out-of-combat heal — free and abundant from looting.
    • Swallow is your go-to in-combat potion. You start with the recipe. Upgrade it as soon as possible.
    • Meditation fully restores health on Story and Sword difficulty. It does NOT heal on Blood and Broken Bones or Death March.
    • Quen Active Shield is the best combat survival tool in the game — absorbs hits and staggers enemies when upgraded.
    • Ekimmara Decoction gives lifesteal — every hit you land restores health passively.
    • The Gourmet skill is the single best early-game value pick — one skill point makes food healing last 20 minutes.
    • On Death March, meditation does not restore health. You rely on Swallow, food, Quen, and decoctions entirely.
    • Patch 5.01 (October 8, 2026) fixed three survival-relevant bugs: the Blade Oils auto-apply feature, excessive stamina drain when sprinting and dodging together, and a delayed dodge response. See the patch notes section below for details.

Healing in The Witcher 3: Wild Hunt Remastered is not as simple as pressing a single button. Geralt does not automatically regenerate health in combat. You need to actively manage your healing across several different systems — and the options that work on normal difficulty are completely different from what keeps you alive on Death March.

This guide covers every healing method in the game, how each one works, how to unlock them, and which methods matter most on each difficulty. It reflects the launch version of Patch 5.0 (September 29, 2026) plus all post-launch fixes through Patch 5.01 (October 8, 2026).

How to Use Healing Items — The Basics

Before anything else, here is how to actually use healing items in Remastered.

    • Open your inventory and go to the Food and Drink tab or the Potions tab.
    • Select the item you want and assign it to your consumables slot.
    • In combat, press Up on the D-Pad (controller) to use your assigned food or drink item. Hold Up to switch between assigned items.
    • Press Down on the D-Pad to use your assigned potion. Hold Down to switch between potions.
    • On PC with a keyboard, healing items are used via the quick item slots — assign them from inventory and use the bound key in combat.

You can assign one food item and one potion at the same time. Both can be active simultaneously. Food and Swallow potion stack — you can eat food while Swallow is running for maximum healing.

Note for new players: Remastered added a Potions and Food tutorial that appears when you first open the Elixirs and Food tab. If you didn’t see it, this was a bug fixed in Patch 5.01 — it should now appear correctly.

Method 1 — Food and Drink

Food and drink are the most accessible healing method in the game. They are everywhere. Every house you loot, every bandit camp you clear, every merchant you visit has some form of food available. Wolves and deer drop raw meat. Beehives around Novigrad drop large quantities of honey. You will rarely run out if you loot regularly.

Food heals slowly over time rather than all at once. You eat or drink something and your vitality bar ticks upward for several seconds. This makes food better for topping up between fights rather than recovering in the middle of one — though it can still help in drawn-out encounters if you are not taking heavy hits.

Different food items heal different amounts and at different rates. Cooked meals and higher quality food restore more vitality over a longer period than raw ingredients. Alcohol works the same way as food for healing purposes, and it also refills your potion charges when you meditate.

The Gourmet Skill — One Point That Changes Everything

Early in the game, one of the best single skill point investments you can make is the Gourmet skill in the General branch. Gourmet extends the duration of food healing effects from a few seconds to 20 full minutes. Every time you eat or drink anything, you get a healing buff that ticks continuously for twenty minutes regardless of what you are doing.

This transforms food from an emergency heal into a passive vitality regeneration system. One piece of bread gives you twenty minutes of healing. On early difficulties, this is close to a free health regen toggle. On harder difficulties, it supplements your potion strategy without any ongoing cost beyond looting normally.

Gourmet is particularly powerful on Blood and Broken Bones and Death March because meditation does not restore your health on those difficulties. Any passive healing that does not cost potions or charges becomes significantly more valuable.

The Witcher 3 Remastered How to Heal Swallow Potion

    Method 2 — Swallow Potion

Swallow is your primary in-combat healing potion. Geralt starts the game with the recipe already unlocked. Drinking Swallow provides vitality regeneration for a set duration — it does not top you up instantly, but it keeps you recovering while you fight.

Swallow adds 20 toxicity per dose. You can hold a limited number of charges based on your current potion tier. When you run out, meditating with any strong alcohol in your inventory refills all your potion charges.

    Swallow Tiers

    • Swallow — The base version. Available from the start. Heals 40 vitality immediately and 40 more per enemy killed during the effect. Lasts 20 seconds.
    • Enhanced Swallow — Crafted upgrade. Stronger regeneration, longer duration, higher charge limit (up to four). Requires 1x Swallow plus additional ingredients.
    • Superior Swallow — The top tier. Can be purchased from Yolar at Gedyneith at level 20 or above. Strongest regeneration, highest charge limit (five). At this tier, Swallow healing continues uninterrupted even while taking damage — which is the key upgrade for sustained in-combat use.

Upgrading Swallow as early as possible is one of the highest-value investments in the game. The uninterrupted healing on Superior Swallow makes combat healing dramatically more reliable against heavy-hitting enemies.

White Raffard’s Decoction — The Emergency Button

White Raffard’s Decoction is a separate potion from Swallow that deserves its own mention. It is the only potion in the game that restores a large chunk of vitality instantly rather than over time. The Superior version fully restores all vitality in one drink and briefly grants damage immunity. Use it when you are at critical health and need to survive a hit rather than regenerate through damage.

White Raffard’s Decoction is expensive to craft and limited in charges. Keep it as an emergency button rather than your regular combat heal.

    Method 3 — Meditation

Meditation is the simplest healing method on lower difficulties — and completely useless for healing on the hardest two.

On Story and Sword! difficulty: meditating for any duration fully restores Geralt’s vitality to maximum. No cost, no ingredients, no limit. You can heal to full whenever you are not in combat simply by opening the meditation menu and waiting a moment.

On Blood and Broken Bones! and Death March! difficulty: meditating does NOT restore health. You will not gain a single point of vitality from meditating on these difficulties, regardless of how long you wait. Meditation on higher difficulties is only used to refill potion and decoction charges (which requires strong alcohol in your inventory) and to pass time.

This is the single most important thing to understand about difficulty in The Witcher 3. If you were healing through meditation on normal difficulty and then switch to Death March, you lose your primary healing method entirely. Everything else on this list becomes critical on harder settings.

Remastered’s updated meditation system now shows time passing with a cinematic timelapse effect rather than a UI screen. The mechanic itself works the same way — the presentation is just more immersive.

    Method 4 — Quen Sign

Quen is not a healing potion, but it is the most important tool for staying healthy in combat. It is a protective shield that absorbs a set amount of damage before breaking. Every hit that Quen absorbs is damage that your vitality bar does not lose.

Keeping Quen active essentially extends your effective health pool by absorbing one or more hits per shield charge. On Death March where every hit hurts significantly, Quen between offensive exchanges is the foundation of survival.

Active Shield — The Upgraded Quen

The Active Shield upgrade in the Signs skill tree transforms Quen from a passive damage absorber into an active healing tool. When Active Shield is equipped:

    • Holding down the Quen cast input creates a maintained shield rather than a single absorb.
    • The maintained shield drains stamina continuously but absorbs all incoming damage while active.
    • When the shield breaks or you release it, it staggers nearby enemies.
    • Any damage absorbed by the Active Shield is converted back into vitality — effectively turning blocked hits into healing.

Active Shield is the best passive healing tool in the game for Signs-focused builds. It is especially powerful on Death March because the healing it provides does not consume toxicity or potion charges. The cost is stamina, which regenerates on its own.

For full details on building around Quen and Signs for maximum survivability, see our best builds guide.

    Method 5 — Decoctions

Decoctions are stronger and longer-lasting than potions, but they cost significantly more toxicity and last until you meditate (which removes them). They are not emergency heals — they are passive systems that change how combat works while they are active.

The two most important decoctions for healing are:

    Ekimmara Decoction — Lifesteal

Effect: Damage dealt to enemies regenerates your health. Every sword hit returns a portion of the damage as vitality.

Ekimmara turns your offensive output into a healing engine. The more aggressively you fight, the more you heal. On enemies with lower armor, fast attacks proc this constantly and keep your vitality climbing throughout a fight. Combined with a Combat or Alchemy build, Ekimmara effectively makes sustained combat self-sustaining.

To get the Ekimmara Decoction recipe, find the book My Evening with a Vampire in a chest near the Orphans of Crookback Bog in Velen. You also need an Ekimmara Mutagen, which drops from Ekimarra monsters found in Velen.

    Ekhidna Decoction — Stamina-Based Healing

Effect: Every action that consumes stamina — casting a sign, dodging, rolling, parrying — restores vitality while this decoction is active.

Ekhidna turns your entire combat movement into passive healing. Every dodge heals you. Every sign cast heals you. Every roll heals you. For Signs builds especially, casting Quen, Igni, and Yrden repeatedly in combat means you are constantly recovering health as a side effect of your normal playstyle. Combined with Ekimmara, these two decoctions together create a near-constant healing loop in active fights.

Patch 5.01 note: At launch, a bug caused Geralt’s stamina to be fully drained when sprinting and dodging at the same time. Because Ekhidna heals on stamina consumption, this bug could cause unintended large healing spikes while also leaving Geralt unable to cast Signs or dodge again until stamina refilled. This bug was fixed in Patch 5.01 (October 8, 2026) on PC, PS5, and Xbox Series X|S. Nintendo Switch 2 players should be aware this fix has not shipped to that platform yet.

    Troll Decoction — Out-of-Combat Regen

Effect: Regenerates vitality continuously over time, similar to food but stronger and without requiring you to eat anything.

Troll Decoction is best used between fights to top up passively while exploring between encounters. It is weaker than Ekimmara in active combat but does not require you to land hits, making it reliable for all situations.

The Witcher 3 Remastered How to Heal Alchemy Skills
The Witcher 3 Remastered How to Heal Alchemy Skills

    Method 6 — Alchemy Skills

Several skills in the Alchemy branch directly improve your healing output without requiring extra potion charges.

    • Refreshment — Restores vitality each time you drink any potion, not just healing potions. Drinking Tawny Owl or Thunderbolt for combat buffs also heals you passively. This is one of the most efficient healing upgrades in the game because it does not consume additional charges.
    • Delayed Recovery — Potion effects do not begin fading until a potion is at maximum effect. Spreads out Swallow’s healing over a longer window, which means more total healing per charge in longer fights.
    • Tissue Transmutation — Converts excess toxicity above a threshold into bonus vitality. Useful for Alchemy builds running multiple decoctions simultaneously, as high toxicity becomes extra effective health rather than a liability.
    • Endure Pain — Converts incoming spike damage into a temporary vitality regeneration buff instead. Smooths out burst damage situations where a single heavy hit would otherwise kill you, by turning that hit into a healing window.

Method 7 — Sun and Stars Skill

Sun and Stars is a General branch skill that provides slow vitality regeneration during the day when out of combat, and slow stamina regeneration at night when out of combat. It is a free passive effect that does not require potions, food, or charges.

Sun and Stars is a convenience skill rather than a primary healing method. It will not save you mid-fight and regenerates too slowly to matter in serious encounters. But it does mean that exploring during the day passively tops you up between encounters without spending food. It is worth taking if you have a spare General skill slot and are not investing it elsewhere.

    How Healing Changes by Difficulty

Story and Sword! (Easy and Normal)

Meditation fully restores health for free. Food and Swallow supplement this. You are unlikely to struggle with healing at these difficulties. Focus on learning combat and the quest systems rather than managing your vitality bar.

    Blood and Broken Bones! (Hard)

Meditation no longer heals. You must rely on food, Swallow, Quen, and decoctions. The Gourmet skill becomes immediately more valuable. Keep Superior Swallow upgraded and carry enough food to top up between fights. Ekimmara Decoction is worth unlocking early for passive lifesteal.

    Death March! (Hardest)

Every hit deals significantly more damage. Meditation does not heal. Every healing resource matters. The recommended approach:

    • Take Gourmet immediately — one skill point for 20-minute food healing.
    • Keep Quen active at all times during fights. Use Active Shield once unlocked.
    • Run Swallow as your primary combat potion. Upgrade to Superior Swallow as soon as possible.
    • Carry White Raffard’s Decoction for emergencies.
    • Use Ekimmara and Ekhidna Decoctions once unlocked — they keep you healing through normal combat actions.
    • Invest in Refreshment and Delayed Recovery in the Alchemy tree for maximum value per potion charge.
    • Enable Automatically Apply Blade Oils in Settings if you use Protective Coating — and make sure you are on Patch 5.01 or later, as this feature was broken at launch and only applied oils if you had enabled it during that session without restarting.

For a full survival-focused build designed specifically for Death March, see our best builds guide which covers the Death March build in detail.

The New Vitality Regeneration Boost Accessibility Option

Patch 5.0 added a dedicated Accessibility section to the Settings menu. One of its options is a Vitality Regeneration Boost, available as part of the Bard’s Ballad accessibility preset. It lets players set an automatic health regeneration threshold — activating below a certain percentage of vitality — and control the rate at which vitality increases.

This option is available in the Accessibility settings menu added in the Remastered update and can be customised independently from the difficulty setting. It is designed for players who find survival management stressful or inaccessible. You do not need to lower the difficulty to use it — it can be combined with any difficulty level including Death March.

For everything else that changed in Remastered including the rebuilt skill tree, see our Patch 5.0 full changes guide.

Post-Launch Patch Notes — Healing and Survival Fixes

CD Projekt Red has shipped several updates since the September 29, 2026 launch. Below are every fix relevant to healing, stamina, combat survival, and Blade Oils — the mechanics covered in this guide.

    Hotfix 5.00c — October 1, 2026 (PC only)

No changes to healing, stamina, or oils. Focused on save file issues, the Classic version launcher, and visual/performance fixes in Velen.

    Patch 5.00b — October 1, 2026 (PS5 & Xbox Series X|S)

No changes to healing, stamina, or oils. Stability and performance focused.

    Patch 5.01 — October 8, 2026 (PC, PS5, Xbox Series X|S)

Important: Patch 5.01 is not yet available on Nintendo Switch 2. CDPR has confirmed it is working on a Switch 2 update to bring feature parity. Switch 2 players remain on the launch build.

Survival and healing-relevant fixes in Patch 5.01:

    • Blade Oils auto-apply bug fixed. The “Automatically Apply Blade Oils” feature only worked if it had been enabled during that exact game session. Restarting the game caused it to stop functioning until you manually toggled it again. This is now fixed. If you are running an Alchemy build using Protective Coating, which gives bonus damage based on the oil you have applied, this was directly breaking your damage output on every session restart. Update to 5.01 and confirm the setting is on in the Options menu.
    • Stamina drain bug fixed. Sprinting and dodging simultaneously was fully draining Geralt’s stamina bar in one go. Since stamina powers Signs and dodge rolls, this could leave you unable to cast Quen or dodge immediately after a sprint-dodge, which is a common Death March survival pattern. For Ekhidna Decoction users, the bug also caused an unintended large healing spike from the sudden full stamina drain. This is now fixed.
    • Dodge delay fixed. Dodging had an unintentionally delayed response at launch, making it harder to avoid hits on harder difficulties. The timing is now corrected.
    • Meditation screen input hints. Control hints were briefly flickering on the meditation screen. Minor cosmetic fix, but relevant if you meditate frequently to refill potion charges.
    • Potions tutorial fixed. The in-game tutorial for potions and food didn’t display correctly when switching to the Elixirs and Food tab for the first time. New players who missed it can now encounter it properly.

Quick Reference — Healing Method by Situation

    Frequently Asked Questions

How do you heal in The Witcher 3 Remastered?

There are six main healing methods: eating food or drink, using the Swallow potion, meditating (only restores health on Story and Sword! difficulty), using Quen’s Active Shield to convert blocked damage to vitality, drinking decoctions like Ekimmara for passive lifesteal, and investing in Alchemy skills like Refreshment. Most effective healing combines two or more of these at once.

Does meditation heal in The Witcher 3 Remastered?

It depends on your difficulty. Meditation fully restores health on Story and Sword! difficulty. It does NOT restore health on Blood and Broken Bones! or Death March! On those difficulties, meditation only refills potion charges (requiring strong alcohol) and passes time. It does not give you a single point of vitality back.

What is the best healing potion in The Witcher 3?

Swallow is your reliable in-combat heal. Upgrade it to Superior Swallow as early as possible — at this tier it heals without being interrupted by taking damage. White Raffard’s Decoction is the best emergency button — the Superior version fully restores all health instantly and briefly grants damage immunity.

What is the Gourmet skill and is it worth taking?

Gourmet is a General skill that costs one skill point and extends all food and drink healing effects to last 20 minutes. It is one of the best single skill point investments in the game, especially on harder difficulties where meditation does not restore health. One piece of food gives you 20 minutes of continuous vitality regeneration.

What is the best decoction for healing in The Witcher 3?

Ekimmara Decoction is the strongest combat healing decoction — every hit you land restores health passively. Ekhidna Decoction is excellent for Signs builds — every stamina-consuming action (casting a sign, dodging, rolling) restores vitality while it is active. Running both together creates near-continuous passive healing in active fights. Note: a stamina drain bug affecting Ekhidna was fixed in Patch 5.01 on PC, PS5, and Xbox Series X|S.

How do you heal on Death March difficulty?

Meditation does not restore health on Death March. Rely on: the Gourmet skill for 20-minute food healing, Superior Swallow as your main combat potion, Quen Active Shield to absorb hits and convert blocked damage to vitality, White Raffard’s Decoction for emergencies, and Ekimmara and Ekhidna Decoctions for passive combat lifesteal. The Alchemy skills Refreshment and Delayed Recovery further extend your per-charge healing value. Also enable Automatically Apply Blade Oils in Settings — this was broken at launch and fixed in Patch 5.01.

How do you refill Swallow potion charges in The Witcher 3?

Meditate with any strong alcohol in your inventory. Meditating refills all your potion and decoction charges simultaneously. Strong alcohol is widely available from merchants, innkeepers, and looting. You should rarely run out if you loot regularly while playing.

Does Quen heal you in The Witcher 3 Remastered?

The basic Quen shield absorbs damage but does not convert it to healing. The Active Shield upgrade in the Signs skill tree changes this — any damage absorbed by the maintained Active Shield is converted back into vitality. On Signs-focused builds, Active Shield effectively turns incoming hits into healing as long as your stamina holds.

Was the Blade Oils auto-apply feature broken at launch?

Yes. At launch, the Automatically Apply Blade Oils feature only worked if it had been enabled during that same game session. Restarting the game caused it to stop working until manually toggled again. This was fixed in Patch 5.01 on October 8, 2026, for PC, PS5, and Xbox Series X|S. Nintendo Switch 2 players remain on the launch build pending a future update from CDPR.

Was dodging broken at launch in Witcher 3 Remastered?

Yes. Dodging had an unintentionally delayed response at launch, making it harder to avoid attacks precisely — particularly on Death March difficulty where surviving hits matters most. The dodge timing was corrected in Patch 5.01 on October 8, 2026, for PC, PS5, and Xbox Series X|S.
