What Is the Nameless Hospital in NTE?
The Nameless Hospital is one of the most unique and atmospheric experiences in Neverness to Everness. It is accessed through the Hethereau Municipal Hospital in Illusion Town, and it is linked to the Nightmare-Bound Anomaly Commission. Once inside, you are transported into an Anomalous Realm that plays out like a Resident Evil-style stealth horror — a multi-floor nightmare hospital filled with locked rooms, unsettling puzzles, and an unkillable mummy-type monster that patrols the corridors.
What makes it special compared to other Anomaly Commissions is that it has four separate endings. Each ending reveals a different piece of the story happening inside the hospital, and unlocking all four gives you the full picture of what the Nameless Hospital nightmare is really about. You need to run the hospital four times, once per ending, collecting a different set of clues each time.

How to Access the Nameless Hospital
The Nameless Hospital is not marked like a standard Anomaly Commission. To get inside:
- Unlock the Illusion Town Wertheimer Tower first. The commission only appears on the map after you reveal this district. Need help? Read our guide on unlocking all map areas in NTE.
- Open the Anomaly Commission menu from the World Map and find the Nightmare-Bound commission in Illusion Town.
- Navigate to the Hethereau Municipal Hospital. The entrance is west of the ReroRero Phone Booth near Davidia Avenue.
- Enter the hospital through the open door and go right, then straight.
- After walking a short distance, you will see a blue NPC facing the wall. Interact with them.
- Select Enter to be transported into the Nameless Hospital Anomalous Realm.
One important rule for all runs: The hospital has a patrolling mummy-type monster that cannot be damaged. If it catches you, you are sent back to the starting point. It does not end your run — just wastes time and forces you to walk back. Stay calm, move quietly, and time your movements around its patrol route.
How the Endings Work
Every ending follows the same final step: collect three specific clues and insert them into the three cameras in the camera room on the second floor. The type of clues you insert determines which ending you see. The endings are mutually exclusive per run — you can only get one ending per visit to the nightmare.
Each clue type is found in different areas of the hospital. The floors and rooms you need to visit change depending on which ending you are going for. Once you place all three clues in the camera room, a cutscene plays and you are returned to the real world.
After completing Ending 1 for the first time, you will always start every future run already holding the Staff Elevator Card. This key activates the elevator, giving you immediate access to the second and third floors from the start. This makes all subsequent endings significantly faster to reach.
Ending 1 – The Price of Betrayal (Anomaly Disorder Clues)
This is the first and easiest ending. It is also the one required to complete the Nightmare-Bound Anomaly Commission from the Illusion Town commission list. All three clues are found on the first floor.

How to get the three Anomaly Disorder Clues:
- Clue 1: From your starting position, walk forward to the nearby front desk. Interact with one of the computers there to collect the first Anomaly Disorder Clue.
- Exit the room and follow the corridor. Watch the monster’s movement — it will go through a door ahead. After it passes, follow through that same door and continue navigating the first floor corridor.
- Clue 2: Keep moving through the corridor. Do not enter every room — follow the path and eventually enter a room on the right. Investigate the computer inside to get the second clue.
- Continue through subsequent rooms. Navigate through the surgical room and keep following the left side corridor until you see blood marks on a wall. Take the door on the left to find another computer. Interact with it for the third clue.
- After getting the third clue, search a nearby blue locker for the Spare Key: 1F. Break the obstacle blocking the door on the left, exit through it, and go upstairs to the second floor.
- On the second floor, move toward the front desk. Find the door beside it and go through. You will reach the camera room with three recording devices.
- Interact with each camera and place one Anomaly Disorder Clue in each. After placing all three, the cutscene for Ending 1 — The Price of Betrayal plays.
Completing this ending also fulfills the Nightmare-Bound Anomaly Commission, giving you all its one-time rewards. From this point on, every future run starts with the Staff Elevator Card already in your inventory.

Ending 2 – Surveillance Record Clues
The second ending requires collecting three Surveillance Record clues. The first is found on the first floor and the other two are on the second floor. Since you now start every run with the Staff Elevator Card, you can reach the second floor quickly via the elevator.

How to get the three Surveillance Record Clues:
- From the starting room, exit and follow the corridor. The monster will enter a room — it is the room directly in front of the elevator. Follow into that room after the monster passes.
- Clue 1: At the end of that room, find a small desk. Interact with it to collect the first Surveillance Record Clue.
- Head back out and take the elevator to the second floor using your Staff Elevator Card.
- From the second floor’s main lobby, walk to the corridor at the far end. Keep moving forward — you may encounter monsters in this section. Fight or avoid them.
- Clue 2: Follow the corridor along the same wall. Just past the bathroom, find a door on that same wall. Go inside. The second Surveillance Record Clue is in this room.
- Clue 3: Continue along the corridor to find the second floor generator room. On the wall inside, you will spot the third Surveillance Record Clue.
- Head back to the camera room on the second floor. Place all three Surveillance Record Clues in the cameras to trigger Ending 2.
Ending 3 – Anomaly Experiment Clues
The third ending requires three Anomaly Experiment clues, all found on the third floor. This is the first ending that takes you into the upper areas of the hospital.

How to get the three Anomaly Experiment Clues:
- Use the elevator to go directly to the third floor from the start.
- On the third floor, you will find two locked gates. The keys are placed right next to each gate — they are not hidden. Target the door on the right side first.
- Find a clock nearby. The key to the right-side door is hidden inside the clock. Take it and open the door.
- Move to the right side to open a new room. This room has a timer — you must find a key hidden in the front lockers before the timer expires. If you fail, the room kicks you out. Take the key and exit through the door on the other side.
- Turn right immediately after exiting. Enter the next room — it also has a timer and some unsettling effects while you are inside. Cross to the other side of the room and find a file with the first Anomaly Experiment Clue.
- Continue navigating the third floor corridor through subsequent rooms. Collect the second Anomaly Experiment Clue from a room further along the path.
- The third Anomaly Experiment Clue is in another room on this floor. Follow the corridor until you find it.
- Once you have all three, take the elevator back to the second floor and place the clues in the camera room. This triggers Ending 3.
Ending 4 – The Hidden Ending (Light Switch Puzzle)
Ending 4 is by far the hardest and most complex of all four. It requires collecting one hidden clue and two specific Anomaly Experiment clues — but getting the hidden clue requires solving a secret puzzle and reaching a room that is not accessible through normal means. You must have Esper Zero or Nanally in your team for this ending.

How to get the clues for Ending 4:
- Start the run and head straight to the elevator. Go to the second floor.
- From the second floor lobby, follow the corridor past the main lobby area.
- Find a room with a middle table. Look for a sealed canister in an adjacent room — specifically the canister that transforms a creature called Mysty into Jigsaw. Interact with the last canister to trigger this transformation.
- Take the Jigsaw back to the middle table and insert it. This adds a door to the room that leads to the last section of this floor’s corridor.
- Follow the corridor from that new door all the way to the end. A new door has appeared on the wall there, connecting the two sides of the third floor together.
- Getting Clue 1 (Anomaly Experiment Clue III): After entering through the connecting door, find a room just before the end of the corridor on the right side. Interact with the files on the desk inside to collect Anomaly Experiment Clue III.
- Getting Mysty / Clue 2 setup: Exit that room and immediately enter the room across from it. At the end of this room, you will find Mysty — an experimental object that can transform. Ask Mysty to come with you and exit back to the corridor.
- Turn left after leaving the room and keep following the corridor. Take a right turn and continue forward. Just before going full circle, find another room. Enter it and interact with the final canister to switch Mysty into Jigsaw form again.
- Go back to the middle table and insert Jigsaw again to open the passage.
- Getting the hidden Clue (Anomaly Experiment Clue VI) — Light Switch Puzzle: In the middle of a specific room on this floor, find a light switch. You need to press it in a very specific pattern to unlock a hidden chamber:
- Press the switch twice quickly.
- Press once, wait one second.
- Press once, wait one second.
- Press once, wait one second.
- Press once, wait one second.
- Press the switch twice quickly.
- If done correctly, a hidden chamber unlocks. Inside is Anomaly Experiment Clue VI — the third and final clue for Ending 4.
- Important — Getting the first Jigsaw from a hidden room: Ending 4 also requires an earlier step where you reach a sealed room above a normal accessible area. The only way to reach it is by using Esper Zero’s Skill to launch upward, then instantly swapping to another character and double-jumping mid-air to reach the higher floor. Alternatively, Nanally’s zero-gravity movements work as well. Inside that higher room is the most important Jigsaw piece — without it, the middle table interaction does not unlock the secret passage needed for this ending.
- Once you have all three clues, go back to the camera room on the second floor and insert them to trigger Ending 4.
Tips for Navigating the Nameless Hospital
- Always complete Ending 1 first. This permanently unlocks the Staff Elevator Card for all future runs, which cuts navigation time in half by giving you immediate elevator access from the very start of every run.
- The monster cannot be killed. Do not waste time or skills trying to fight it. Time your movements around its patrol and slip past it quietly. If it catches you, you just restart from the beginning of that floor — you do not lose your clues.
- Target one ending per run. Each ending requires different clues in different areas. Trying to collect clues for multiple endings in one run wastes time and increases your chances of getting caught by the monster.
- Ending 4 requires Esper Zero or Nanally. Bring one of them in your team before attempting the fourth ending. Without their special movement abilities, the hidden room is inaccessible.
- Memorize the layout early. The hospital layout stays the same every run. After your first two visits, navigation becomes much faster as you learn which corridors lead where.
- Grab the safes when you are already inside. Two safes are hidden in the Nameless Hospital Anomalous Realm. If you are running the hospital for endings anyway, grabbing them during your runs is an efficient way to collect free Fons. See our safe locations guide for their exact spots.
- The generator is optional. You may notice generator puzzles in various rooms. Activating the generator provides power to certain areas but is not required for any of the four endings. Skip it to save time.
- How to activate the generator (optional): If you want to explore fully or need power for a specific puzzle — activate the middle generator in the back first, then activate the one in front of it. This sequence starts the power correctly.
Rewards for Completing All Endings
Completing all four endings is one of the most rewarding content loops in Illusion Town. Here is a summary of what you get:
- Ending 1 (first-time completion) gives the full Nightmare-Bound Anomaly Commission rewards: Hunter Level EXP ×450, Annulith ×60, Fons ×20,000, Silver Carrota ×160, U-00NE ×40, Beetle Coin ×10,000, Anomaly Material Selection Box I ×6, and the permanent Staff Elevator Card.
- Endings 2, 3, and 4 each give their own reward payouts for completing additional story content.
- All four endings together reveal the complete story of what happened inside the Nameless Hospital — one of the most detailed and creepy pieces of lore in the game.
- The two hidden safes inside the Anomalous Realm add extra Fons on top of the commission rewards.
