TL;DR
- MW4 uses a Pick 3 system โ one perk per slot, unlocked at Player Level 4 (Create-a-Class).
- There are 18 perks total โ 6 per slot.
- Best Perk 1: Ghost (stealth) or Quick Fix (aggressive)
- Best Perk 2: Ninja (footsteps) or Fast Hands (handling)
- Best Perk 3: Cold-Blooded (detection counter) or Battle Hardened (tactical resistance)
- Best all-round combo: Ghost + Ninja + Cold-Blooded or Quick Fix + Fast Hands + Battle Hardened
Perks have shaped Call of Duty loadouts since the original Modern Warfare, and MW4 keeps it simple โ three slots, one choice each, and 18 perks to pick from. The right combination quietly decides dozens of gunfights per match. The wrong one leaves you visible on the minimap or caught reloading at the worst moment.
This guide covers every perk, what it does, when it unlocks, and which ones are worth running in each slot.
How the Perk System Works in MW4
MW4 uses the classic Pick 3 perk system โ one perk from each of three slots per loadout. You can’t run two Perk 1 options in the same class, so every choice has a genuine opportunity cost. Create-a-Class unlocks at Player Level 4, and the two default perks in each slot are available immediately from that point.
Perk 1 covers detection, stealth, and survivability. Perk 2 covers economy, information, and handling. Perk 3 covers stamina, equipment resistance, and tracking. Building a strong class means filling each slot with something that works toward the same goal rather than pulling in three different directions.

Best Perks in MW4 โ Slot by Slot Breakdown
Best Perk 1: Ghost vs Quick Fix
Ghost is the most universally valuable Perk 1 in the game. Staying off enemy UAVs removes the single biggest information advantage opposing teams can hold. In modes where UAVs are called every 30 seconds โ which is most modes at mid-to-high skill lobbies โ Ghost keeps you off the minimap and in control of your own position. The downside is the unlock level: it sits at Level 24, so you’ll be using something else early.
Quick Fix is the better aggressive pick. Immediately triggering health regen after a kill keeps you healthy through multi-kill sequences and objective pushes without retreating to cover after every fight. It’s available at Level 4 and is the strongest default Perk 1 while you’re grinding toward Ghost.
High Alert has a niche โ it pulses your vision when an enemy outside your view is aiming at you, which helps on open maps with long sightlines. Its weakness is that Cold-Blooded in Perk 3 cancels it entirely, and enough players run Cold-Blooded that you’ll frequently get no value from it in competitive lobbies.
Best Perk 2: Ninja, Fast Hands, or Blood Rush
This is the most contested slot in MW4. Three perks genuinely compete for it depending on playstyle.
Ninja at Level 30 is the best overall Perk 2 for players who flank and push off-angles. Silent footsteps remove one of the most reliable ways enemies locate you before they see you. On maps with tight corridors and hard angles, Ninja wins more gunfights than almost any attachment change could. It’s a late unlock though โ you’re grinding to Level 30 before it’s available.
Fast Hands at Level 20 is the most broadly useful Perk 2 across all weapon types. Faster reloads and weapon swaps reduce the two most dangerous windows in any gunfight. It works well on every weapon class and requires no playstyle adjustment to get value from.
Blood Rush is the pick for pure run-and-gun play. Tactical Sprint bursts let you close distances faster, and kills refreshing your stamina mean you stay at peak speed through an entire engagement chain. It’s available at Level 4, making it one of the strongest early-game options in the entire perk list.
Best Perk 3: Cold-Blooded vs Battle Hardened
Cold-Blooded is the best Perk 3 in MW4. It blocks detection by AI Targeting Systems, Thermal Optics, Recon Drones, Spotter Scopes, Snapshot Grenades, and โ critically โ High Alert. That last point matters a lot: enemies running High Alert in Perk 1 get no warning when you’re looking at them from outside their view. Cold-Blooded effectively negates one of the most common Perk 1 choices in the game. It unlocks at Level 36.
Battle Hardened unlocks early at Level 12 and reduces the effect of enemy flash and stun grenades. In a game where Tactical grenades are thrown constantly, this keeps you functional through pushes that would otherwise stop you cold. It’s the best Perk 3 option while you’re grinding toward Cold-Blooded, and remains a strong second choice at any level.
Best Perk Combinations for Every Playstyle
Best All-Round Stealth Build
Ghost + Ninja + Cold-Blooded
The classic stealth trio. Ghost keeps you off UAVs, Ninja keeps your footsteps quiet, and Cold-Blooded blocks every AI detection system in the game including High Alert. This is the combination to run when you want full information control โ enemies genuinely struggle to locate you through any electronic means. All three are later unlocks (Level 24, 30, 36), so it’s a grind to complete.
Best Aggressive Slayer Build
Quick Fix + Fast Hands + Battle Hardened
Built for pushing fights and keeping momentum. Quick Fix keeps you healthy after back-to-back kills, Fast Hands keeps your weapon ready at all times, and Battle Hardened stops Tacticals from breaking your flow when you push a room. Two of the three are available at Level 4, making this the strongest early-game perk setup in the game.
Best SMG / Mobility Build
Quick Fix + Blood Rush + Battle Hardened
Designed for constant movement. Blood Rush’s Tactical Sprint burst keeps you fast between engagements, kills refuel your stamina so you stay quick, and Quick Fix means you don’t need to stop moving to regen health. Works best with fast-firing SMGs. See our best SMGs ranked guide for which weapons suit this playstyle.
Best Killstreak Farmer Build
Kill Chain + Hardline + Battle Hardened
Kill Chain feeds half your Killstreak kills back into your next streak โ chain your streaks back-to-back and they become self-sustaining. Hardline cuts the cost of every streak by one kill, lowering the entry point for your entire chain. Battle Hardened keeps you alive through the Tactical spam that comes once enemies see streaks going up. High risk, high reward. Check our full killstreaks guide to plan which streaks to build around.
Best Objective / Team Play Build
Quick Fix + Bomb Squad + Scavenger
Quick Fix triggers on objective interactions, not just kills โ capturing and holding objectives keeps your health topped up constantly. Bomb Squad reduces incoming grenade damage when you’re holding a point, and Scavenger keeps your ammo refilled from enemy drops so you’re never caught empty on an objective.
Best Counter-UAV Build
Ghost + Birdseye + Cold-Blooded
Ghost removes you from enemy UAV sweeps entirely. Birdseye extends your own counter-UAV radius and shows enemy directions on pings. Cold-Blooded blocks AI detection systems and High Alert. This combination is built for players who want full information dominance โ knowing where enemies are while staying invisible yourself.
Best Perks for Early Game (While Levelling Up)
Not every best perk is available from the start. Here’s what to run while you grind toward the top-tier options:
- Level 4: Quick Fix + Blood Rush + Battle Hardened โ this trio is strong immediately and stays competitive even after you unlock later perks
- Level 12: Quick Fix + Blood Rush + Battle Hardened (Battle Hardened now available)
- Level 20: Quick Fix + Fast Hands + Battle Hardened โ Fast Hands is a genuine upgrade over Blood Rush for most weapon classes
- Level 24: Switch Quick Fix to Ghost as your Perk 1
- Level 30: Ghost + Ninja + Battle Hardened โ the stealth build starts coming together
- Level 36: Ghost + Ninja + Cold-Blooded โ the full stealth combo is now complete
Tips for Getting the Most From Your Perks
- Build perks around your weapon, not the other way around. An SMG class wants Blood Rush or Ninja. A lane-holding AR class wants Ghost and Cold-Blooded. Match the perks to the weapon’s engagement range. See our best multiplayer loadouts guide for full class setups.
- Cold-Blooded blocks High Alert. If you see enemies running High Alert in Perk 1, Cold-Blooded in Perk 3 completely neutralises it. A common enemy perk becomes a wasted slot.
- Ghost doesn’t help you when you’re moving. You need to be moving or firing for Ghost to fully suppress UAV detection in some modes. Know when the UAV is in the air and adjust accordingly.
- Ninja is the most underrated perk. Most players don’t realise how often footstep audio gives away their position. Ninja removes that tell entirely โ enemies can’t react to sound they can’t hear.
- Grind Double XP early. Getting to Level 24 for Ghost and Level 30 for Ninja is the real barrier to the best perk builds. Check our guide on how to get Double XP Tokens in MW4 to speed that up.
