What Is the Nightmare-Bound Anomaly Commission in NTE?
Nightmare-Bound is one of the most unique and atmospheric Anomaly Commissions in Neverness to Everness. Located in the Illusion Town district, it sends you into an Anomalous Realm hidden inside an ordinary hospital building. Once inside, the game transforms into a stealth horror experience — dark corridors, locked rooms, unsettling puzzles, and an unkillable monster patrolling the halls.
What sets this commission apart from all others is its four separate endings. Each ending requires a different set of clues collected from different floors of the hospital. You need to run the Nameless Hospital four times, with each run targeting one ending. The full story of what happened in this place only becomes clear once you have seen all four.

How to Unlock Nightmare-Bound
Like all Anomaly Commissions, Nightmare-Bound becomes available after you clear the Main Story Prologue – Part 2. Once unlocked, open your World Map and access the Anomaly Commission menu from the bottom-right corner. Navigate to the Illusion Town section and find Nightmare-Bound in the list. Click Track Anomaly to mark the hospital on your map.

Where Is the Nameless Hospital?
The Nameless Hospital Anomalous Realm is accessed through the real-world Hethereau Municipal Hospital. This building is located on Onion Road in Illusion Town, just west of the ReroRero Phone Booth near Davidia Avenue. The entrance to the hospital is right across from the Phone Booth — it is an open door you can walk straight through.
Once inside, walk straight through the lobby and turn right toward the door with the stairwell. Head up the stairs and look for a blue-tinted NPC standing in the corner, staring at the wall. Interact with them and select the “Close Eyes” dialogue option. This transports you into the Anomalous Realm.

Key Rules of the Nameless Hospital
Before going in, there are a few important mechanics to understand. These apply to every run, regardless of which ending you are targeting.
The Patrolling Monster
A bandaged, mummy-like monster patrols the corridors of the hospital. You cannot damage or defeat it. If it spots and catches you, you are sent back to the starting point of that run. Your clue progress is saved, so nothing is lost — it just forces you to walk back to where you were. Stay calm, watch its patrol route, and time your movements around it.
When the monster is nearby, hide behind large objects like shelving units, power generators, or room dividers. Wait for it to move on, then continue. Do not sprint past it unless you are certain it is looking away — the running animation can draw its attention.
The Camera Room Is Always the Final Step
Every ending follows the same conclusion. Once you collect three clues of the required type, head to the camera room on the second floor (behind the main desk) and insert all three clues into the projectors. A cutscene will play and you will exit the Anomalous Realm.
The Staff Elevator Card
Completing Ending 1 for the first time gives you a permanent Staff Elevator Card. From your second run onwards, you always start with this card already in your inventory. It activates the elevator in the hospital, giving you immediate access to the second and third floors from the very beginning of each run. This makes all subsequent endings much faster to complete.
The Generator Power Puzzle
During Ending 1, you will need to restore power by activating four generators in a specific order. The correct sequence is:
- First: Activate the middle generator in the back row
- Second: Activate the generator directly opposite it
- Third: Return to the front row and activate the unit by the window
- Fourth: Activate the one next to the front door
Activating them in the wrong order does not work. When done correctly, the power turns on and the Bandaged Monster spawns into the room. Use the generators as cover and slip past it carefully to continue.
Ending 1: The Price of Betrayal
This is the first and easiest ending. It is also the one required to officially complete the Nightmare-Bound Anomaly Commission. All three clues are Anomaly Disorder Clues, found on the first floor.
- Clue 1: From the starting position, walk to the nearby front desk and interact with one of the computers.
- Clue 2: Exit the starting room and follow the corridor. Watch the monster — it will enter a door ahead. After it passes, follow through that same door. Continue navigating the first-floor corridor and locate the second clue.
- Clue 3: Continue following the first-floor path. After picking up the second clue, navigate through the next set of rooms to find the third Anomaly Disorder Clue. Note that after taking this clue, a mummy-type monster spawns. Exit through the next room rather than backtracking to avoid it.
Once you have all three clues, collect the Keychain: 1F from a nearby cabinet, clear the trash blocking a door, and use the stairs to reach the second floor. Enter the camera room behind the main desk and insert all three clues into the projectors. A cutscene plays, you exit the Anomalous Realm, and Ending 1 is complete.
This also awards you the Staff Elevator Card permanently.
Ending 2: An Offering to Evil
For the second run, you are collecting Surveillance Record Clues. The first is on the first floor and the other two are on the second floor. Since you now have the Staff Elevator Card, you can reach the second floor immediately after picking up Clue 1.
- Clue 1: Exit the starting room and follow the monster into the room directly in front of the elevator. Navigate through the shelves to find the first Surveillance Record Clue at the end of that room.
- Clue 2: Take the elevator to the second floor. From the second floor’s main lobby, head to the corridor at the far end. Go left and enter the first room after the bathroom. The second clue is inside.
- Clue 3: Follow the same corridor and locate the second floor’s generator room. Inside on the wall, you will find the third Surveillance Record Clue.
Note: Two different monster variations can spawn on the second floor. Werewolf-type monsters react to light; floating creatures flood the floor with water and may open certain doors. Either variation allows you to complete Ending 2. Return to the camera room and insert all three Surveillance Record Clues to unlock Ending 2.
Ending 3: The Third Run
For the third run, you are collecting Anomaly Experiment Clues, all found on the third floor. Use the Staff Elevator Card at the start and go straight to the third floor.
- On the third floor, you will find two locked gates. The keys are placed right next to each gate.
- Gate 1 (right side): The key is hidden inside a nearby clock. Open the door and enter the next room. Find the second key in the front lockers before a timer runs out. Proceed through to the next timed room and collect Anomaly Experiment Clue 1 from a file on the far side. Continue through the next room where monsters spawn — defeat them and collect Anomaly Experiment Clue 2.
- Gate 2 (left side): The key sits in a canister right next to this gate. Enter, open the door on the right, and navigate to the room at the end of the blocked corridor. Anomaly Experiment Clue 3 is inside.
Take the elevator back to the second floor and insert all three Anomaly Experiment Clues into the camera projectors to unlock Ending 3.
Ending 4: The Voice of the Nightmare (Final Ending)
Ending 4 is the hardest and most story-rich run in the Nameless Hospital. It requires either the main character (Esper Zero) or Nanally in your team — make sure one of them is active before starting this run.
This ending involves a hidden clue that requires interacting with a specific room mechanic — a light switch puzzle that opens a secret passage containing a unique clue. You then combine this with two specific Anomaly Experiment Clues gathered from rooms on the upper floors. The puzzle involves manipulating in-room switches in the correct sequence to open a concealed chamber.
Collect the required three clues, return to the camera room on the second floor, and insert them in the correct order to unlock the fourth and final ending — “The Voice of the Nightmare”. This ending completes the full story of the Nameless Hospital and reveals what really happened at Coldmount Clinic.

Tips for Surviving the Nameless Hospital
Getting Caught Is Not the End
When the monster catches you, it sends you back to the starting room. It does not reset the run or remove your clues. Your progress is saved. Just walk back to where you were and continue. Stay calm — panicking and running often makes things worse.
Watch the Monster’s Route Before Moving
The monster follows a predictable patrol pattern in most areas. Before crossing a corridor or entering a new room, stop and watch where it is heading. Wait for it to turn away or enter another room, then move quickly and quietly.
Use Large Objects for Cover
Rows of shelves, power generators, large desks, and room dividers all block the monster’s line of sight. If it is getting close and you have no time to exit a room, duck behind the nearest large object and wait. The monster will usually continue its patrol and move on.
Do Not Backtrack After Picking Up a Clue
In several rooms, picking up a clue triggers a monster spawn nearby. The game design pushes you to exit through the next door rather than backtracking. Always look for an exit on the far side of the room after grabbing a clue. Going back the way you came puts you directly in the monster’s path.
Use the Elevator Every Run After Ending 1
Once you have the Staff Elevator Card, always use the elevator at the start of each run instead of the stairs. It gives you immediate access to the floor you need for that ending and cuts the time spent navigating the first floor significantly.

Completing Ending 1 for the first time fulfills the Nightmare-Bound Anomaly Commission and earns you the following one-time rewards:

These are some of the best one-time rewards available from any Anomaly Commission in the game. The Annulith and Fons alone make this one of the highest-value commissions to complete early. Endings 2, 3, and 4 each provide their own separate reward payouts on top of this.
